The Structured Query Language is the sole culprit of mankind’s inability to tame and properly design large-scale databases, if only because prolonged exposure is rumored to cause irreversible mental damage and eliminate the ability to visualize information as anything but a set of interconnected flat tables.

Also, SQL Server is the first thing that comes to mind.
